Download the order forms you need that match your transfer requirements. Please read carefully before using.
A complete Shipping Kit is made up of a shipping instruction sheet, a contact sheet (w/ signed Term and Conditions), applicable order form(s) and a shipping label – with or without a Commercial Invoice if shipment is international. All of these documents can be downloaded here from this page.
The Order forms you download here are made up of several sections, one, the shipping instructions that meet both local or international requests, two, will have the applicable order form for the media transfer you are interested in and three, our standard Terms and Conditions. Fill in what you can or want, along with your signature on the Terms and Conditions sheet. Fill in any additional information you feel will make your order a successful delivery. Use extra paper if need be. Please read all of the enclosed documents carefully to ensure an accurate description of your request is conveyed. Please write or check your selections clearly.
International orders will require a Commercial Invoice if you plan to use a bonded courier like FedEx or UPS (recommended). A blank form is available for download. The local FedEx or UPS store can help you fill out these forms. Declare a value of less than $20 total for your shipment(s). Used video cassettes and old film reels have no real monetary value in the eyes of the shipping services and in precedent. Liability is limited to media item replacement only – not content.
The Commercial Invoice requirement can be avoided if you use the postal service or use our free Shipping Kit service (shipping forms will already be filled out for you). If you plan to use the postal service please make sure you use an expedited level of service that provides insurance and a tracking number. International orders using the Postal Service is not really recommended. Tracking services if necessary to exercise are more difficult once your package crosses the border.

Standard Definition Film Transfer Forms – Archival Method

Choose this form if you wish to transfer your family films (8mm, Super 8 or 16mm with or without audio) to standard DVD and/or computer editable DV AVI files. The transfer will be done in Standard Definition – SD (720×480) using our Archival frame by frame telecine system. Cost is $0.25/ft with the option of color/density correction for an additional $0.10/ft – 8mm films are notorious for shifting colors. Audio based films add $0.10/ft for re-synchronization. PAL and Quicktime conversions available for an additional $0.03/ft. See our SD film transfer page for more information.
DVD copies are $9.95 per disk. Volume discounts will apply. DV AVI files can also be written to Archival Gold DVD’s at $16.95 per disk for small film transfer runs.
A hard drive is highly recommended for large film transfer orders requiring DV AVI video format. A USB-2 type packaged drive will do. At 13GB per hour of film, choose your hard drive size accordingly. As a measuring stick, about 1000ft of 8 or Super8 film is an hour of playtime. Account for an additional 3 times working space for your edit sessions.
High Definition Film Transfer Forms – Archival Method

Choose this form if you wish to transfer your films (8mm, Super 8 or 16mm with or without audio) to High Definition on Blu ray disk or to DVD in AVCHD and/or computer editable 4:2:2 YUV QT files. The transfer will be done in High Definition (1920×1080 or 1280×720 sizes) using our archival frame by frame telecine system. Costs can range from $0.28/ft to $0.49/ft depending on what you wish to have done. Audio based films add $0.10/ft for re-synchronization. Quicktime conversions available. Blu ray copies are $19.95 per disk, DVD AVCHD are $9.95 per disk. Volume discounts will apply.
A hard drive is highly recommended if you wish to have the various HD files offered in selected HD transfer bundles — YUV QT video a bare 1TB SATA drive (min) is a must for about 1000ft of film. Please no USB packaged drives. At about 150GB per 400ft reel, choose your hard drive size accordingly. Account for an additional 3 times working space if the drive is to be part of your edit sessions vs just for archive.

Videotape transfer Order Forms

Choose this form if you wish to transfer your video tapes to DVD and/or computer editable DV AVI files. We will transfer, VHS, S-VHS, VHS-C, 8mm, Hi8, Digital8, UMatic, DVCAM, Mini-DV and Betamax in either standard NTSC or PAL format. The transfer will be done in Standard Definition (720×480) using our professional video capture stations. Costs are $17.95 per running hour of video. There is a $35 minimum. We do a standard run of edits for you at no additional cost – blue screens, leaving camera on, etc. if required.
DVD copies are $9.95 per disk. Publishing to Archival grade DVD’s are available at $16.95 per disk. Volume discounts will apply.

35mm Slide transfers to DVD and Slide Shows Order Forms

Choose this form if you wish to transfer your 35mm slides or film strips to DVD. Slide scanning can be done in a number of resolutions from 1000dpi to 4000dpi. Costs can range from $0.35 (1000dpi) to $1.49 (4000dpi) each depending on scan resolution. We also scan photos. See our Slide Transfer page for more information.
DVD copies are $9.95 per disk. Volume discounts will apply. Archival Gold grade disks are $16.95 per disk as an option.
Your slides can also be made into a self running slide show or video that will play on a standard DVD set top player. The slide show can have appropriate mode music included if you choose. It can be a great way to review your slides in a more relaxing setting. Slides shows start as low as $39.95.
A hard drive is optional. A standard USB-2 type drive will work or a USB flash drive.
Commercial Invoice for International ShipmentsChoose this form if you wish to make out your own shipping documents that meet international shipping standards/requirements. Print this form out and fill out as suggested below…
In the upper left hand block just enter the shipping date and the waybill number from your shipping form from USP or FedEx. The rest is left blank. The block below “Ship to” fill in the shipping details for HomeDVD. The block in the upper right “Shipper” is you. Fill in your address details. Don’t need to fill in Tax/VAT, Company or email information. The block below that – “Sold to”, is HomeDVD information again.
The shipping content needs to be filled in next. Just enter the number of reels or tapes etc you have, and the vlaue for each one – $1 or less each. Make sure the total when all is said and done is not more than $20. Weigh your package and enter into provided space. In the Additional Comments and Declaration Statement – leave empty. That’s it.
Make three copies and sign\date each one. Include these 3 copies in the shipping pouch along with the appropriately filled out waybill. Keep a copy of your waybill for tracking purposes if need be.
Make sure the declared value of the goods is below $20. Unfortunately shipping companies do not place any value on these old “cost to replace” goods, even though they are very valuable to you.

Contact Information with Terms and Conditions of Engagement
This document is very important to include in any shipment to us. It will have all of the necessary contact information and the terms of our contractual engagement. Please read the Terms and Conditions part and sign your acknowledgement.
There are sections for return shipping choices and shipping dates. Please make your choices and wishes known. All shipments received will be inventoried and documented. An email acknowledging your shipment to us and a short summary of what you would like done will be sent to you for confirmation. Any changes must be indicated before work begins.
